COMPANY PROFILE:
This innovative, technology-forward medical practice that specializes in remote patient monitoring and chronic care management in the Fort Lauderdale Florida area in 2020 and is currently hiring due to rapid growth. Care is coordinated with the support of dedicated assistants, allowing the providers to give their patients attentive and full care via 100% telemedicine. After an initial telemedicine evaluation, patients are sent equipment and detailed instructions, so that they can monitor their chronic conditions at home, with data sent to the provider’s office for analysis and care planning.
POSITION DESCRIPTION:
This position relies heavily on technology, on both the parts of the provider and the patient. Patients are seen via telemedicine and providers work out of the Fort Lauderdale office with on-site support staff. The company greatly values clinicians with many state licenses as they have patients across the country.
While adjustments are made as needed, the typical process for care is:

  1. – physician performs initial evaluation and determines conditions to monitor
    – patient is sent all necessary equipment and instructions
    – office regularly receives data on patients and support staff triages with providers
    – nurse practitioners and/or RNs continue regular care monitoring, follow-up visits, etc.
    The role of the RN is to serve as a primary care support provider for patients, many of whom are geriatric.
    Providers see an average of 8 to 10 patients per day, but it’s at the provider’s discretion to determine frequency and need of visits, based upon the diagnostic reports being received from the home health monitoring equipment.
    The new RN will work collaboratively in a team approach with NPs and/or physicians to determine who sees the patients and how often.
    Visits are typically done on a proactive monthly basis, and then as needed based on the diagnostic reports. Telemedicine encounters average 20 minutes in length.
    The practice uses an extremely user friendly proprietary EHR.
