Registered Nurse Remote Team Lead Care – TX, NY, and/or Compact Licensure

About the role:

The Team Lead / Care Manager will provide remote therapeutic monitoring services to patients, with a specific focus on monitoring — and helping to manage patients’ medication adherence in a remote, virtual-care setting. This person can be remotely located but must be registered in Compact states and NYS. This role will lead the first Care Team, composed of LPNs and RDs, to support in-hospital care plans, beginning with Total Joint Replacement. 

 

As a Team Lead Care, this person will begin by assisting with the care delivery strategy, implementing the program with our initial client, and administering digital care plans for Remote Patient Monitoring, Chronic Care Management. As the Care Team Leader, this role will coordinate patient care, including billing activities for RPM, CCM, and other codes as needed when added to the program and will assign Medical Nutrition Therapy work to Registered Dietician(s). 

 

Responsibilities:

  • Work with the team on the design of care plan content, delivery plan, and platform.
  • Interact with clinicians and hospital staff during program implementation and ongoing management.
  • Deliver patient-facing care to support Remote Patient Monitoring and Chronic Care Management billing codes.
  • Deliver prepared content to patients on schedule according to care plans.
  • Establish a process to deliver clean billing reports
  • Along with the Registered Dietician, learn about the product benefits as added to the Standard of Care and communicate these to patients as part of the patient journey.
  • Work with the Care Team to establish methods for sharing learnings, best practices, and patient feedback for continuous improvement.
  • Responsible for triaging patients and knowing when to escalate to clinicians or transfer to technical support or other resources.
  • Work proactively to identify improvements in patient engagement, satisfaction, and billing opportunities.
  • Managing or supporting initiatives that contribute to the program and company goals
  • Collaborate with Product, Research, Sales, and Marketing teams on strategies, programs, and patient communications to ensure consistency of messaging and voice.
  • Monitor Key Performance Indicators for the program’s success and, using reporting dashboards, regularly update Leadership on the status.
  • Hire and train new team members reporting to you.

 

 

Skill requirement:

  • Familiarity with Remote Patient Monitoring and Chronic Care Management software platforms.
  • 5+ years of clinical practice experience, ideally in a hospital or large medical group.
  • 3+ years in management and leading.
  • Ability to interact with technology, including wearable devices (with minimal technical support).
  • Knowledge of CMS requirements.
  • Being comfortable in the start-up environment where changes happen frequently.
  • Ability to practice at the top of the RN license in hands-on remote patient care across chronic conditions.
  • Awareness of requirements for nursing support of MSK / acute orthopedic indications.
  • Experience working with at least one Electronic Healthcare Records / Electronic Medical Record (EHR/EMR).
  • Motivational interviewing experience is preferred, but knowledge and willingness to learn are acceptable.
  • Willingness to learn new skills fast.
  • High level of patient empathy, particularly for an older population.
  • Data-driven with the ability to connect results with goals.
  • Organized and self-starting, creative problem solver.

 

Education/certifications:

  • This person MUST be licensed to work in Texas (or other compact states) and be willing to obtain additional state certifications.
  • Preferred location for this role would be either the NYC Metro area or the Dallas Metro area with hybrid remote work.
  • CCM/RPM certification is a pulse.
  • Program Management Certification is preferred.
  • Advanced business degree is preferred.
