Windstone’s Health & Wellness Screening Program is a proprietary screening service designed to proactively detect behavioral health diagnoses which have previously gone unrecognized.

• Become a part of a fantastic team of nurses screening health plan members who may need behavioral help before a crisis.
• Work in a call center atmosphere assessing the clinical status of patients referred by our Healthcare contracts.
• Through a brief scripted screening, the Screener will assist in the detection of at-risk patients for behavioral health related conditions.
• If the screening indicates the possible presence of a behavioral health condition, the Screener will be responsible for scheduling an in-home assessment by a licensed clinical practitioner.

***Physical demands of this position require the ability to sit for long periods of time, on the phone and working at a keyboard.

Additional Requirements:

Program: EmpowerDX, a telemedicine consultation by phone for patients with positive COVID-19 results

Qualifying Providers: MDs, NPs, PAs

Payment: $17.00 / consultation

Communication: Voice / phone

Program Description:

The EmpowerDX program provides patients who take an at-home COVID-19 test and receive a positive or inconclusive test result with a mandatory telemedicine follow-up visit by phone. 

Jointly run under SteadyMD and Nurse-1-1, this program provides a supervising physician through SteadyMD if needed, professional liability insurance, and payment of $17 per successfully completed patient consultation or $10 if unable to connect with the patient. The work can be done remotely from home. Each provider has full control over their availability and when they choose to answer a consult request. MDs, NPs and PAs will be conducting synchronous phone visits to discuss positive test results and provide guidance for asymptomatic and symptomatic patients experiencing mild, moderate and severe symptoms. The program does not require prescribing of any kind. Calls are expected to take 5-10 minutes.

Provider Qualifications:

• NPs with an active NPI and state license to practice clinical care with prescriptive authority in any of the following states: Montana, Nebraska, Alaska, Arkansas, Connecticut, Delaware and Rhode Island. Nurse practitioners must have a minimum of 3 months of clinical experience
• PAs with an active NPI and state license to practice clinical care with prescriptive authority in any of the following states: Montana, Nebraska, Arkansas, Connecticut, Delaware and Rhode Island. Physician assistants must have a minimum of 2 years of clinical experience.
• Each provider will be hired as a 1099 contractor and sign a Professional Services Agreement with SteadyMD. If a provider has already signed a PSA with SteadyMD previously and PSA is still active, they will not be required to sign another contract. The Care Coordinator operates under the HCA Health Home Care Coordination Organization and the designated Health Home Lead. The Care Coordinator directly interacts with Health

Home service beneficiaries and is responsible for the conversion of beneficiaries from outreach to engaged.

The role of the Care Coordinator involves direct coordination, advocacy, and education to assist the beneficiary in understanding the healthcare system and to access services for physical and behavioral health needs.

Essential Position Functions:

• Coordinates and oversees Health Home benefit services by
actively engaging beneficiary via completion of Health Action Plan and supporting the beneficiary to achieve their short and long-term goals.
Responsible to maintain adequate number of beneficiaries currently outreaching to, necessary to maintain a caseload of 55.

• Meets with beneficiary on monthly basis and/or
actively assisting and coaching beneficiary on action steps related to long and short-term goals.

Responds to individual cases per Health Home scope of services, e.g., follow-up with beneficiary in the hospital prior to discharge or within two days of discharge in the event beneficiary is hospitalized.

• Review Health Action Plan every four months with each
beneficiary to maintain eligibility for services.

Marginal Functions of the Position:

• Complete projects and tasks as assigned by supervisor;
