Nurse Practitioner in Psychiatry Role Description:
We are seeking a Nurse Practitioner in Psychiatry. This role would be a flexible
contracting role – working between 5 to 20 hours a week at a rate of $90-$100 dollars
per hour – providing quality care for clients who are suffering from mental health
concerns. This position has the opportunity to turn into a full-time staff position with

The role:

Health coaches employed by Ventricle Health will be the face of the company’s brand and will manage most of the members’ interactions with our products and services. Responsibilities

  • Provide real time response and support for members (including new member intake, triage, education, reminders, vitals monitoring, etc.) on a shift basis with coach team.
  • Learn and implement clinical best practices in collaboration with a broader care team
  • Actively engage with members and fellow clinical staff via tools including email, text, audio/video call and software applications for instant messaging and clinical note taking.
  • Attend meetings and trainings online and/or in person when required. Prepare training documentation and train new health coaches as the team grows and expands.
  • Follow best practices for scheduling on-call hours and efficiently meet member needs

  • Acts as liaison for involved parties in the worker’s compensation injury by communicating in person and telephonically with the patient/caregiver, attorneys, employer, claims examiners, telephonic nurse case manager, return to work specialists, vocational rehabilitation supervisor, provider(s), ancillary provider(s) and health care team.
  • Monitors injured employee’s progress towards appropriately established outcomes through continued assessment and evaluation.
  • Coordinates necessary referrals, consultations, and therapeutic services for the injured employee.
  • Completes the interview process with the patient/caregiver in the home, worksite, ancillary location or physician’s office for current and ongoing assessments and evaluations.
  • Assists in the development of continued care plans as necessary to include patient education and prevention programs.
  • Testifies as needed to substantiate relevant case work or reports.
  • Confers with medical treating providers concerning the course of care and treatment per clinical guidelines.
  • Educates medical providers on appropriate disability and treatment guidelines as necessary.

Education & Licensing

In addition, the NN will conduct educational programs within HCP offices, conduct peer to peer educational programs, and provide patient follow up pre and post infusion and injection training within their assigned territory. The NN will utilize using their professional expertise, relationships and clinical training in the implementation of an education plan.

  • Provide in person and virtual support to patients newly prescribed Product for Crohn’s Disease/UC
  • Maintain flexibility to vary assignments whether telephonic, in-home or video injection training in order to accelerate disease state awareness for patients and care givers.
  • Integrate compliantly, with sales professionals, reimbursement specialists and other matrix partners with the goal of optimizing patient experience.
  • Master all aspects of the disease state and knowledge of patient resources as it applies to Crohn’s and Ulcerative Colitis medications.
  • Ability to work within complex technologies with ability to complete/balance several tasks at once maintaining focus and accuracy in order to provide patient support through the approved, designated call cadence.
  • Acts as liaison to various patient resources pre and post infusion and throughout the patient’s participation in the NN program. Follow up via telephone/video support to patients undergoing treatment for Crohn’s Disease/UC.
  • Accurately and timely documents all interactions with patients and HCPs daily and completes all reports and written assignments in a timely, accurate and thorough manner

Job Requirements

Hybrid Remote Uniform Assessment System (UAS) RN – New York

Job Description – UAS Assessor:

Hamaspik Choice, MLTC is seeking to hire a Nurse Assessor. The Nurse Assessor works for the Managed Long Term Care Plan conducting UAS-NY assessments and reassessments to evaluate potential members for enrollment eligibility in the MLTC Plan. Clinical assessment skills are critical. Position entails collaboration with the Utilization Management team, members and family members in providing the best possible individualized care.

Responsibilities include:

  • Conducts UAS-NY reassessment visits
  • Post visit communication with members of care management and care planning teams.
  • Initiates development and coordination of a safe plan of care with member/family to meet all members’ needs; coordinates with rest of care management team.
  • Documentation of UAS-NY and visit notes including progress note and plan of care
  • Participates in the agency QA Program, as requested.
  • Performs assessments and completes documentation within the time frames set forth by the Agency
  • Participates in mandatory staff meetings

Specific knowledge, skills, and Abilities:

  • Strong organizational & communication skills
  • Computer proficiency
  • Ability to work autonomously and as part of a team

Business Experience:

  • Minimum 1 year of MLTC/ UAS-NY experience

Licenses/Certification:

  • NYS RN License
  • Drivers License
